Do you not know your new address yet? Please inform us of the country you are moving to.

Copy your ID
Make a copy of your valid passport, Dutch identitycard or your residence permit.

Send the form and a copy of your passport, Dutch identity card or residence permit to:

Your departure will be registered in the Personal Records Database (BRP) within 5 weeks.
